We are seeing totally different things.

I still get my summary.

Mine does,  In fact, I've been thinking about adding some classes for
the new menu because some of my color combinations are not nearly as
graphically pleasing :-) on a wide, variable length bar down the left 
side of the screen but it works as expected and is easy to fix.

 | 3) There is no way to tell which menus have submenus (expand)

I agree.  Maybe a standard "expand" icon is needed.

 | 4) There is no space/offset in the main frame left-hand side.  If it
 |    honored the themes, this might not be a big deal.  But since it doesn't
 |    honor themes, the main frame text (etc) is right up against the menu
 | frame
 |    edge and it looks "cramped" (due to different colors in the frames).

 |    this probably wouldn't look bad if the themes (backgroup colors) matched.

Since my themes are working, I was thinking of making the menu bar narrower
on my Mozilla it seems too wide.  I'll add a screen shot to show you what
I see with themes and summary.

I have an embarrassing question.  I haven't been able to group my apps.
I missed something somewhere.  I have added the menu_parent and have
experimented with heading status in registry.php with no luck.  The
only one I have is the hardcoded administration:-(  I should know the
answer, sorry.
