http://server/kronolith/ics.php?c=user%40server, and iCal uses a calendar
name of "Subscribed" and "Subscribed 2" instead of the real names as
specified in Kronolith.  The X-WR-CALNAME header should be added to the
iCalendar output.  eg:

X-WR-CALNAME: US Holidays

Looks like we need to add a way to get the calendar title in Horde_iCalendar
and need to call the setter in kronolith/ics.php
