To reproduce (see, e.g., Bug 4382):
1. add attachment
2. delete attachment
3. add attachment with same name as #1
Now, attachment will be linked in both the new entry AND the entry created
after step #1
4. Deleteing the new attachment will remove the attachment from both
entries.
