One of the changes in 1.96 was to move the Prefs_UI::generateHeader call
from the beginning to the end of the generateUI function. This breaks the
prefs forms, causing the main part of the forms to go before the header,
followed by the rest of the form.

Moving the generateHeader call back to the top of the function fixes the
issue.
