I have the very same problem, with the same error message:

May 10 10:39:20 calendars HORDE: HORDE [horde] Image not found  
[/var/tmp//horde_form_Sa0SPa] [pid 27203 on line 63 of  
"/data/httpd/calendars.ifom-ieo-campus.it/docs/horde/services/images/view.php"]

The ACLs on the LDAP directory are correct, since the exact entry is  
found and the attributes, including jpegPhoto, are successfully read  
from LDAP but the picture is not displayed.
