Solved it - turned out to be a very large EMail (2 attachments, 70MB)  
which broke PHP's memory limit.

PHP Fatal error:  Allowed memory size of 134217728 bytes exhausted  
(tried to allocate 52195185 bytes) in  
/var/www/horde/pear/php/Horde/Stream.php on line 403

putting

memory_limit = 512M

in php.ini fixed it, outlook sync is fine now :)
