  Log:
  Adding a new <eref> entity to the help system.  <eref> can be used to
  embed an external link in a help entry.
  
  This adds a new 'helplink' style, which is not also used by the <ref>
  entities.
  
  Also, use $_SERVER['PHP_SELF'] instead of the global $PHP_SELF.
