  - Add the new Delete ad form
  - Total rewrite of this campaign management script
  - Use use the new $actionID var to process different form requests
  - Add the delete, close, and no delete/close, form submissions
  - Rewrite the new campaign form
  - Use overall the simpler syntax for the form generation ($form->renderActive...)
  ** NOTE: This is dependant on the implementation of the patch submitted for the horde/lib/Form.php libs - to handle the reset button.
