[dev] Gollem Server List Patch

This patch addes server lists nad a prefered mechanism to Gollem and also allows
the user to input ftp server details if the admin allows. Also a conf.xml for
Gollem and a screen shot of the login prompt just for fun.

Most of the file upload checking got removed in manager.php as
it doesn't work under win32 due to forward slash / backslash and dirname()
inconsistancies.

Also adds a bit of documentation in backends.php.dist
along with a couple of examples.
