I should have read this bug report first. ;-)

I experience the extension problem as well, but I think it's not related to
this specific bug because it happens with _every_ attachment, not only these
ones with unknown mime types.

But there is another bug report on bugzilla (a _huge_ one, I guess some
hundred comments) because this (appending the cgi's extension) happens
always on windows. I don't know if your patch fixes this problem, but iirc I
already tried that in the past and it didn't change anything. I'll try it
out anyway.
