[dev] Patch: new Horde_Form type

Hi, list.

Attached are a couple of files that add a new Horde_Form type:  a date field with
an image aside it that brings up a calendar widget which allows the user to
point and click their way to the correct date.  The .js file goes in
horde/templates/javascript/.  This patch also requires that the kronolith icon
(or any appropriate icon, for that matter) be copied to
horde/graphics/calendar.gif.  The calendar javascript is an adaptation of the
one used in Moment (Thanks, Mike =).
