I think this is a good idea, but i'll give you my experience with it.  I run
squid (proxy server) with adzap which basically blocks ads.  It works really
well and by default the image it uses for the replacement says "ad zapped".  The
only problem with this was that if the replacement image is a small image and
its replacing a really large image (defined by the height and width attributes)
the image would be scaled resulting in some ugly graphics.  I switched it to a
clear graphic to get around this (and hence why i used a blank image when i
wrote this up).  I agree that to users this can be deceiving, but whoever
designs the graphic - i have no creative ability so i probably wont' be doing it
:) - needs to take this in consideration.
