[dev] Inconsistant Menus

Playing around with a couple of Horde apps the last couple of days, I've
noticed that some apps (and even parts of the summary) have the app menu
items on the right, and some on the left, because some of them don't
define any menu bar text.

What I think is needed is one standard menu bar, which apps can use API
calls to add items to, so that the menu bar is only defined in one
template, instead of for every app.
