turba of course. that's what that example is about. but you can store that data 
wherever you want - a different DB, a text file, or /dev/null for that matter -
just create your own code to store the data in the
_horde_hook_signup_addextra() function.

and to set up the form you don't _need_ to use the attributes.php from turba,
but just follow the syntax from that file.
