[dev] IMP's fetchmail code

IMP currently has two fetchmail-related UIs - one for fetching mail from an
existing account, another for setting up accounts to fetch mail from.

I'd like to combine these two so that accounts can be set up, saved, and
fetched in one place, via the same UI. Looks like it'll be a simple matter
of merging the two (similar) pages and templates. Thoughts/objections?
