[dev] menu reload

We seem to have lost the ability to reload the menu after logging out when
$conf['menu']['always'] == true.  My guess is that this happened when we moved
to the unified common-footer.inc.  The attached patch re-introduces this
feature in horde/templates/login/login.inc.  I will also look into the imp
login script and submit a patch to that if necessary.
