[doc] upgrade procedure to from 4 to 5

i have execute thie command

/var/www/horde/pear/pear -c /var/www/horde/pear.conf  config-set 
preferred_state beta

and i have tried to upgrade the packages with

  /var/www/horde/pear/pear -c /var/www/horde/pear.conf  upgrade-all

some part of error...
horde/horde is already installed and is the same as the released version 
4.0.15
horde/imp is already installed and is the same as the released version 
5.0.23
horde/ingo is already installed and is the same as the released version 
2.0.9
horde/kronolith is already installed and is the same as the released 
version 3.0.17
horde/nag is already installed and is the same as the released version 
3.0.8
horde/turba is already installed and is the same as the released version 
3.0.15
pecl/memcache is already installed and is the same as the released 
version 2.2.7
upgrade-all failed
