The other one is horde/phplib/local.inc if you're storing PHPLIB session
information in MySQL, but that one wouldn't cause the error you're seeing.  
(The horde/test.php3 "Click here to test PHPLIB" would find that one.)

Have you tried horde/scripts/database/dbpasswd.sh?  It will change all the
passwords for you to make sure they're the same.

Other than that, the standard advice to carefully check the host (FQDN vs.  
localhost matters!), user (hordemgr), and password in the Horde/IMP files
and try to access MySQL from the command-line using that password.
