Does /usr/lib/sendmail actually exist ?  Is it executable by the apache
(or whatever your webserver run as) user ?

Please note that, for example, if you are running Postfix, the sendmail
executable is not located at /usr/lib/sendmail, but at
/usr/bin/sendmail.  Also, if you are using Postfix again, the Pistfix
main.cf config file (located at /etc/postfix/main.cf under RedHat) must
be readable by the user executing the sendmail command.
