[horde] Gollem missing "logout" button

Hello,

I installed Horde and Gollem and want users to have only
access to the Gollem application. 

I have two questions:

1. How can I make Gollem the default application without allowing
the users to change this? As it is now, it looks like each user will have
to go to the "display" option and ask for Gollem as the initial application.

2. Gollem's top menu doesn't seem to include a "logout" button. Is
this a bug? I have "Display logout link for authenticated users" selected
in the "Menu settings" tab.
