[horde] SQL Creation Help Needed PHPMyAdmin

I'm trying to use (create.mysql.sql) and phpmyadmin to created the tables
and grant rights to the DB & Tables But I just cant quite figure out how to
edit create.mysql.sql to do this using phpmyadmin.

Can someone help me get my create.mysql.sql) file correct? My DB Information
is below. Thanks

DB Server Address= dbxxx.perfora.net

DB User Password xxxxxx
