Did you mean that you can login to the IMAP server via telnet using an
account in the MySQL database? If YES, then your IMAP server is
running OK, otherwise you have to fix the IMAP configuration first.

No, you may have to configure LDAP in dovecot if you want to use LDAP.
otherwise it uses system users(AFAIK)

Step 1:
Run http://<yourdomain>/horde/test.php and check whether if you have
any critical errors. If you have any critical errors, fix them before
you proceed.

Step 2:
Run http://<yourdomain>/horde/imp/test.php and do the tests provided.
If you have any critical errors, fix them before you proceed.

Step 3:
Now  complete Horde installation and configure it. The most important
sections are the *General*, *Database* and *Authentication*.

Step 4:
In the *General* section, you have to specify the administrators for
your Horde instance. Add one of the users from your IMAP user
database(I assume it's a MySQL database) to $conf[auth][admins].

Step 5:
Now move on to *Authentication* part and select *IMAP Authentication*
from the drop down labelled $conf[auth][driver].

Step 6:
Save the configuration, and login using the admin account you provided
in Step 4. You should be able to login now.
