[horde] LDAP Hooks - new wiki page

I have added hooks in my CentOS 6 H5 setup to retrieve default email  
address and full name from LDAP. I have documented this  
at http://wiki.horde.org/LDAPHooksHorde5. 

The default hooks in hooks.php.dist did not work for me for various  
reasons, not just localised variables, so I have had to modify them. I  
also wanted to ensure that they were using TLS, which I have now done.

Happy if anyone has any suggestions for improvement, or other comments.
