I just setup a Windows 8.1 preview VM. I was able to successfully
create and use an account with provisioning set to either force or
allow. Windows 8 always seems to send a PROVISION request, even if
it's not prompted to do so by the server, and it doesn't seem to
accept the status code that indicated no provisioning is needed. Not
sure if there is something else going on here, or if Windows 8 mail
*requires* provisioning.
