[imp] external POP3/IMAP access

[Pasi Puentes]

The default login form on the server I configured has three fields on the
form: username, password and language.  On the IMP demo site it has
additional fields: servername, port, mail protocol, and folders.  How can
the standard login form be modified in the same way?
