I notice no DB is listed - are you using shared memory? When I tried
using shared memory on Redhat v7.1 (which also has PHP v4.0.6) I got the
same problem. Apache would hang up so tight the only way to kill it was
to send a SIGKILL (no response to any other signals). 

I would suggest downgrading PHP back to v4.0.4.
