I had this problem not too long ago.  I still don't know what the cause was, but
I was a little behind on some of my apps, so I took the opportunity to compile
up the latest Apache with all the usual toys and the problem went away in the
process.  Perhaps the problem was with (for example) PHP or something, and using
a newer version was what fixed it.
