Importing addressbook from CSV file

I'm trying to configure CSV importing to a LDAP repository.
Hopefully, I have now an empty, writable LDAP database to export to.

Now I need only to know CSV file specifications.
Any reference?

Are entries exported in a per-user fashion or in a single, shared to all
repository?
