Actually, I figured this one out -- at least I did something that worked:

I added the following to horde/turba/templates/menu/menu.inc

/* Logout */
Menu::printItem(Horde::applicationUrl('../imp/login.php?reason=logout'), 
_("Logout"), 'logout.gif',
$registry->getGraphicsPath("horde"));

If you the white page is really empty not just white text on white 
background or an interrupted page that doesn't render probably, there is a 
chance that your webserver segfaults. Check the server logs for any error 
messages. Also the horde log (defaults to /tmp/horde.log) may give some 
helpful information.

Where are you missing a logout button. As you write to the imp list I 
assume you're speaking about imp. You should have at least one logout 
button in the menu.

Lock the folders preference in prefs.php.

There once was such an option but it went away. You can directly access imp 
for now to get to the inbox after login.

That's what the expand names button is for. There also was a patch on this 
list a few days ago that added a button to open a turba window directly 
from the compose window.
