I don't know of the "best combination" but the one single best performance
boost I've found was a php accelerator.  That made more of an improvement
than all the other things I've tried combined. 

Without more info, I can't suggest more, since they would be setup-specific.

I use the same except postgresql instead of mysql.  You'd probably get
much better performance from different mail software than WU-IMAP, but
there are also good reasons not to leave UW-IMAP that you may want to
keep in mind (and why I'm still using also).
