[imp] Using a php page on Redhat to create a user account using openLDAP for IMP 3.2

I need to allow users to come in to our intranet webpage and create a user
account.

I will have a php page with a form on it that asks for:
First Name: txtbox
Last Name: txtbox
Email wanted: txtbox
Password: txtbox
Password confirm: txtbox

Submit: button

Does anyone have a link to a resource that would show me how to work with
openLDAP using php to create a user account in IMP 3.2 via a webpage?
