[imp] Setup Problems

Hey there, Im having problems setting up IMP

When I goto the site at 
http://fidelitysoft.net/mail

I get the following error messages

Notice: Undefined index: alternate_login 
in /home/fidelity/public_html/mail/login.php on line 96

Notice: Undefined index: server 
in /home/fidelity/public_html/mail/login.php on line 143

Im not familiar with PHP enough to know what it wants.

Can someone please assist me with getting my IMP setup?
