I have exactly the same configuration but without any patches, plain 
vanilla, and it works fine.  I think I have never touched it.  The fact 
that you can see it in thunderbird would make me ask about permissions. 
  IIRC your maildirsize is 640 and set to the courieruser.  Can you 
read the maildirsize as the horde user?
