[imp] Undisclosed Recipients in sent-items

I have a user who, when trying to send messages (most often, if not  
only messages with multiple recipients) gets the following headers in  
her sent-items, and the messages don't get sent. No error is  
presented to the user.

From: Disgruntled User <correct at email.com>
To:
Subject: Workshop
MIME-Version: 1.0
Content-Type: text/plain;
	charset=ISO-8859-1
Content-Disposition: inline
Content-Transfer-Encoding: 7bit

There are no recipients in the To: header, but she claims that she  
typed them in, comma separated (or added them from Turba, she can't  
remember.) Looking at her sent-items reveals that this isn't the  
first time the has happened. I'm assuming it's user error, but does  
anyone know what causes it so I can tell her what NOT to do?
