[imp] how to upgrade IPM H3 (4.0.4) --> 4.1

There are no docs in the FAQ for using a straight tarball upgrade (anyone
want to volunteer to write one?)

Note that 4.1 isn't released yet, so you can't really upgrade to 4.1
(but can to 4.1 RC 2).
