[ingo] removing default rule buttons

Ok, my bad.  White/blacklists are controlled by the registry in
ingo/lib/Ingo.php in the getMenu function.  So disabling them in the registry
disables them from the menu.  Vacation/Forward are also controlled from the
getMenu function and that's where I commented out the Forward code (we use
Vacation with Sieve).
