[sam] sam + ldap patch

Here's a start at an LDAP backend for sam.  It needs the spamassassin 
ldap patch at http://bugzilla.spamassassin.org/show_bug.cgi?id=2205 
to be of any use.

Right now the user needs to be able to modify the spamassassin 
attribute of their ldap record (there's no 'rootdn' setting) and 
defaults are configured inside the backend parameters.

It's more-or-less a hack of spamd_sql but so far it seems get the job 
done.
