Okay, well, I'm not sure if this can be done easily or not (though I am
sure it can be done).
The passwd web page at http://www.horde.org/passwd/ lists some poppassd
servers, some of which support PAM. You would then need the PAM module
to do what you want. I'm not sure, but maybe the pam_tally module would
work -- Description is:
# pam_tally: this module keeps track of the number of times an attempt is made
to access an account. It can deny access after a specified number of failures.
Root's account can be treated specially. RPMS for newer versions of this module
than the one contained in the source tree are here:

This might work, or be made to work. You'd need to put in the passwd PAM
setup, not the login one... There may be other (or even better) PAM
solutions -- I didn't do an exhaustive search, I just used some existing
knowledge...
