consolidate display/add/edit includes

There is a lot of duplicating code in the display/add/edit includes.  For
example, I just wanted to change "Alias" to "Nickname".  I had to change it in 3
different places.  I don't know what is best or most efficient but this seems to
take away from the beauty of php.    :)
