[turba] Undefined indexes in Turba latest CVS

[Ibarra, Michael]

When I first enter Turba. The only difference that I have 
vs a straight forward setup is an additional addressbook
in mysql. I've checked the syntax there and it looks identical
to the other mysql portion.
