[ulaform] Unable To Setup

Hello:

	I am trying to get ulaform working but I am having some
difficulty understanding some of the steps outlined in ulaform/
README. I am running the latest CVS as of this AM. From this README: 

(1)  Run the ulaform.sql script (as MySQL root) to create a "ulaform"
database and a "ulaform" user with full rights to it.  This also creates
the tables and populates them with an example form.

OK, I am able to create the table, but where and how do I create a
ulaform user?

(2)  Edit common.php to specify the database host name, user, password,
etc. that will find the database created in step (1).

Where is common.php? What goes in it?
