[ulaform] patch - field ordering

hi ulaformers,

this patch fixes a little feature that's been bugging me for a while -- when you
add fields they seem to be placed in a random ordering position within the
form, and I keep finding myself having to go back to the reordering page to fix
them up. This fixes that by setting the default field order position to be the
end of the list.
