[ulaform] allow user to override 'From' address

hi again,

This updates a patch I sent a while back.

If the option is activated by the site admin, this allows the end-user's email
address to be the address that the mailto form is apparently sent 'From' in the
generated email headers. If the user doesn't provide an email address in the
form, the email headers default to the 'From' address specified by the site
admin.

I'm not sure if this makes any sense to anyone but me, but here's the patch
anyway ;)
