[ulaform] Adding Fields

I'm not sure if im totally missing something here or not, but I've been tracing
through this code for hours and I finally decided to go for help ;-)

I'm trying to add a form field to a form that I have created. However, the only
box that is displaying it "Form Details" from edit.php. "fields.php" appears to
be the correct file to be adding form fields from, however the "Type" combobox
is empty. As far as i can tell its supposed to be populated by
Horde_Form_Types, but it isnt.

Is this a case of the code simply not being finished or am I missing something?
