Yes, I did. This how to reproduce it: Say you have a type and defined
states and priorities for it.
Then you click on "Edit Types", select the type name in the drop down box
and click on the button "Edit Type". You get - or at least I get - a list
of states and priorities for this type but the "Name" and "Description"
fields are emtpy.
The same happens if you click on "Edit States" or "Edit Priorities" and
select one to edit in the next screen. As a result you get an edit form
with empty fields.
Wait, I just recognized that in the page where you select the priority to
edit, the title bar says "Edit Priorities for '' - Stage 1". You see the
empty string where the type name should be?
I should say that I didn't use the sample sql script but created the
modules and types all with the new administration interface.
