[whups] No Stage 4 Form and cannot assign new ticket to user.

I do have individual users assigned Show, Read, Edit & Delete in the module.

I must have something else wrong somewhere...

If I create a New ticket there is no Stage 4 Form to assign it to anyone.

I think this is where my problem actually starts, we are supposed to get a 
Stage 4 form right?

If I click on this new ticket it opens, I click the Assign Ticket link and 
cannot assign it to anyone, users are listed in the 'Assignee' combobox, 
States are now listed in the 'New State' combobox, yet I select a user and 
click Submit the page just blinks.

If I open the ticket as admin and click the Asign Ticket link it sets 
the 'Owner' as the person I just assigned, tickets never show anything in 
the 'Assigned' field.
