[whups] create new tickets newbie question

Hi
I've installed and configured WHUPS finally. Everything works perfectly 
without any warning or errors except for creating new tickets. When I 
try to create a new ticket I get only to step 2 and then after submit I 
am at step 1 without any errors in horde mysql or system logs, and of 
course without any tickets created :(

I've run it on Redhat 7.3, PHP 4.3.7.
Any ideas would be appreciated.
