Ideally there would be a configurable flag (user and system) to write the
total cost of the message (with all recipients) dynamically on the screen as
the message is typed in.

This calculation would need to take into account the following (thinking
clickatell, but other gateways should have similar functionality).

Get and sum credits for each MSISDN.
Multiply by number of SMS's concatenated together (based on number of
character being sent).

Further, with a user configurable preference for currency type, you could
display the cost of the message in local currency (this would require
calling a backend site with a currency rates API, or, doing a daily slurp of
currency rates into the horde backend).
