[dev] order in horde_categories

ok another patch for the categories reorder stuff... it wasn't working 100% last
time, plus there were some parts which needed cleaning up.

if someone could test this out... plus there's also the null driver too. haven't
had a chance to test it out in a meaningful way so if anyone has a proper null
driver install anywhere it would be great to get some feedback...

one thing that i couldn't figure out, and again would be good to hear some
feedback, i am sorting the results by category order at the sql level. but then
it all goes to waste when the extractAllLevels routine is used and mangles up
the whole sorting.

is it better to:
a) stick with sql sorting and modify extractAllLevels to handle the sort orders
better
OR
b) forget about sql sorting completely and rework extractAllLevels to be aware
of the sort orders.

thoughts?
