[dev] horde signup

made a rough start to a signup of users in horde.

anyone have any comments on this approach?

signup.php processes the request:
 - checks if the auth backend has add capabilities
 - and if user self-signup allowed then inserted into the db
 - if not then queued in horde_categories for the admin to approve the request
